Creatine after menopause: new meta-analysis shows gains in muscle mass and strength
Creatine is often seen as a supplement for young strength athletes. New scientific research, however, shows that creatine can also be of interest for women after menopause.
At this stage of life it becomes increasingly important to actively maintain muscle mass and strength. A large meta-analysis from 2026 shows that creatine monohydrate, especially when combined with resistance training, can provide valuable support.
Seven randomized trials with 608 women
The researchers analyzed seven randomized, placebo-controlled trials including a total of 608 postmenopausal women. The average age of participants was about 62 years.
The studies lasted between 12 and 104 weeks, with a median duration of 38 weeks. This allowed researchers to examine not only short-term effects but also changes that developed over a longer period.
The primary outcomes were:
- Change in fat-free mass;
- Maximal muscle strength;
- Physical functioning;
- Safety and tolerability.
Participants used creatine monohydrate, with or without resistance training.
On average, increased fat-free mass
In five studies with a total of 338 participants, body composition was measured precisely using a DXA scan.
Women who used creatine gained on average 0.37 kilograms more fat-free mass than the control group. The confidence interval ranged from 0.05 to 0.69 kilograms.
Fat-free mass is a commonly used measure to track changes in body composition and muscle mass. A difference of 0.37 kilograms may seem modest, but it can be relevant during aging. The goal is not only to build muscle, but also to counteract natural decline as much as possible.
The authors therefore describe the observed effect as small but meaningful.
Clear improvement in leg strength
The effect on muscle strength was even more striking.
In three studies with 111 participants, maximal performance on the leg press improved by an average of 7.5 kilograms more with creatine than in the control group. The confidence interval ranged from 2.2 to 12.8 kilograms.
Bovendien waren de resultaten van deze studies onderling zeer consistent. Dat versterkt het vertrouwen dat creatine daadwerkelijk iets kan toevoegen aan de effecten van krachttraining.

Wat gebeurt er rond en na de menopauze met spiermassa en spierkracht?
Tijdens en na de menopauze daalt de productie van oestrogeen. Deze hormonale verandering heeft niet alleen invloed op bekende overgangsklachten, maar kan ook gevolgen hebben voor de spieren en de lichaamssamenstelling. Vrouwen kunnen geleidelijk spiermassa en spierkracht verliezen, terwijl de hoeveelheid lichaamsvet juist kan toenemen en de vetverdeling kan veranderen.
Deze veranderingen ontstaan niet uitsluitend door de menopauze. Ook leeftijd, lichamelijke activiteit, voeding en leefstijl spelen een rol. De afname van oestrogeen kan het natuurlijke verlies van spiermassa echter versterken. Hierdoor kan het na de menopauze moeilijker worden om spiermassa te behouden of opnieuw op te bouwen.
Waarom is spierbehoud na de menopauze zo belangrijk?
Sterke spieren zijn belangrijk voor veel meer dan alleen sportieve prestaties. Ze ondersteunen de botten en gewrichten en spelen een belangrijke rol bij balans, mobiliteit, stofwisseling en het zelfstandig kunnen uitvoeren van dagelijkse activiteiten.
Het behouden van spiermassa en spierkracht kan helpen om lichamelijke achteruitgang te beperken. Krachttraining is daarom juist tijdens en na de menopauze belangrijk. Creatine is not a replacement for resistance training, but research shows that creatine monohydrate, especially in combination with resistance training, can have additional benefits. It can help postmenopausal women improve or preserve muscle strength, muscle mass, and physical performance.
Creatine and resistance training enhance each other
The most convincing results were found when creatine was combined with resistance training.
In studies that used at least 5 grams of creatine per day in combination with resistance training, improvements in fat-free mass and muscle strength were most pronounced. Studies with lower doses without resistance training found no measurable effect.
That underscores an important principle: creatine is not a substitute for exercise, but can support the body's response to resistance training.
During short, intense efforts muscles use quickly available energy. Creatine plays a role in making this energy available again. This can help creatine enable a bit more strength or training volume during workouts. Over the long term, that can contribute to better training results.
The combination of creatine, sufficient protein, and regular resistance training is therefore an interesting strategy for maintaining and building muscle after menopause.
Positive safety profile
Safety findings were also reassuring.
Reported side effects were mostly mild and occurred about as often in the creatine groups as in the placebo groups. In addition, no adverse changes were found in the kidney values examined.
This aligns with the broader scientific literature in which creatine monohydrate is known as one of the most extensively studied sports supplements.
People with an existing medical condition or specific medication use should of course discuss supplement use with their doctor.
Why creatine monohydrate?
The studies in this meta-analysis examined creatine monohydrate. This is the most researched form of creatine and the form with the strongest scientific support.
Other, often more expensive forms of creatine have so far not convincingly shown they are more effective than creatine monohydrate.

Conclusie
De nieuwe meta-analyse van zeven gerandomiseerde studies en 608 postmenopauzale vrouwen laat zien dat creatine monohydraat, vooral in combinatie met krachttraining, de ontwikkeling van vetvrije massa en spierkracht kan ondersteunen.
Gemiddeld werd 0,37 kilogram extra vetvrije massa en 7,5 kilogram extra vooruitgang op de leg press gevonden ten opzichte van de controlegroepen. Tegelijkertijd waren de gemelde bijwerkingen mild en vergelijkbaar met placebo.
Creatine monohydraat blijkt daarmee niet alleen een supplement voor sportprestaties, maar ook een interessante aanvulling voor vrouwen die hun spieren en fysieke kracht tijdens het ouder worden actief willen ondersteunen.
