Creatine after menopause: new meta-analysis shows gains in muscle mass and strength

Creatine is often seen as a supplement for young strength athletes. New scientific research, however, shows that creatine can also be of interest for postmenopausal women.

At this stage of life it becomes increasingly important to actively maintain muscle mass and strength. A large meta-analysis from 2026 shows that creatine monohydrate, especially in combination with strength training, can provide valuable support.

Seven randomized studies with 608 women

The researchers analyzed seven randomized, placebo-controlled trials with a total of 608 postmenopausal women. The average age of participants was about 62 years.

The studies lasted between 12 and 104 weeks, with a median study duration of 38 weeks. This allowed researchers to examine not only short-term effects but also changes that developed over a longer period.

The primary outcomes were:

  • Change in fat-free mass;
  • Maximal muscle strength;
  • Physical functioning;
  • Safety and tolerability.

Participants used creatine monohydrate, with or without resistance training.

On average, more fat-free mass

In five studies with a total of 338 participants, body composition was measured precisely with a DXA scan.

Women who used creatine gained on average 0.37 kilograms more fat-free mass than the control group. The confidence interval ranged from 0.05 to 0.69 kilograms.

Fat-free mass is a commonly used measure to track changes in body composition and muscle mass. A difference of 0.37 kilograms may seem modest, but it can be relevant during aging. The goal then is not only to build muscle, but also to counteract natural decline as much as possible.

The authors therefore describe the observed effect as small but meaningful.

Clear improvement in leg strength

The effect on muscle strength was even more striking.

In three studies with 111 participants, maximal performance on the leg press improved on average 7.5 kilograms more with creatine than in the control group. The confidence interval ranged from 2.2 to 12.8 kilograms.

Moreover, the results of these studies were very consistent with each other. That strengthens the confidence that creatine can indeed add something to the effects of resistance training.

Strong leg muscles are important for many everyday movements, such as:

  • Climbing stairs;
  • Getting up from a chair;
  • Walking and covering longer distances;
  • Sports and other physical activities;
  • Maintaining mobility and independence.

Creatine is therefore not only interesting for sports performance, but also for women who want to age actively and remain physically strong.

Creatine and resistance training reinforce each other

The most convincing results were found when creatine was combined with resistance training.

In studies that used at least 5 grams of creatine per day in combination with resistance training, improvements in lean mass and muscle strength were most evident. Studies with lower dosages without resistance training found no measurable effect.

This underscores an important principle: creatine is not a substitute for exercise, but can support the body's response to resistance training.

During short, intense efforts, muscles use quickly available energy. Creatine plays a role in making this energy available again. As a result, creatine can help deliver a little more strength or training volume during workouts. Over the long term, that can contribute to better training results.

The combination of creatine, adequate protein, and regular resistance training is therefore an interesting strategy for maintaining and building muscle after menopause.

Positive safety profile

Safety outcomes were also reassuring.

Reported side effects were predominantly mild and occurred at about the same rate in the creatine and placebo groups. In addition, no adverse changes were found in the kidney values examined.

This aligns with the broader scientific literature in which creatine monohydrate is known as one of the most extensively studied sports supplements.

People with a pre-existing medical condition or specific medication use should, of course, discuss supplement use with their doctor.

The studies in this meta-analysis examined creatine monohydrate. This is the most studied form of creatine and the form with the strongest scientific support.

On average, an additional 0.37 kilograms of fat-free mass and 7.5 kilograms greater improvement on the leg press were found compared with control groups. At the same time, reported side effects were mild and comparable to placebo.

Creatine monohydrate therefore appears not only as a supplement for athletic performance but also as an interesting addition for women who want to actively support their muscles and physical strength as they age.
